Your main responsibility is to correctly enter payments, adjustments, co-insurance, deductibles, contractual write-offs and denials from health insurance payers and patients into billing systems, while RCM Managers, Billing Directors, Financial Controllers and Practice Managers set up high-level operational policies, fee schedules, bank reconciliation procedures and financial performance frameworks. Responsibilities Electronic Remittance Advice (ERA) and Electronic Funds Transfer (EFT) Posting: Import, examine, and handle automated ERA files from insurance payer portals and clearinghouses; confirm electronic payments with bank deposit notifications and resolve ERA posting exceptions. Explanation of Benefits (EOB) and Patient Payment Posting: Enter 100% accurate data into Practice Management (PM) software by manually posting paper checks, credit card payments, lockbox deposits, patient co-pays, deductibles, and co-insurance. Contractual Adjustment & Write-Off Verification: In accordance with payer-specific contracts and billing procedures, correctly apply contractual allowances, fee schedule adjustments, write-offs, non-covered costs, and line-item payment distributions. Denial Tagging & Secondary Claim Escalation: Use the proper CARC and RARC tags to identify, document, and forward claim rejections, partial payments, and zero-pay remittances to the A/R Caller and Denial Management teams for prompt follow-up. Batch Reconciliation & Zero Balancing: To guarantee there are no financial disparities, perform daily financial balancing by comparing posted batch totals to real bank deposits, lockbox statements, and clearinghouse summary reports. Skills: Deep practical understanding of medical billing operations, EOB/ERA structures, EFT reconciliation, lockbox procedures, CARC/RARC rejection reason codes, and patient responsibility computations is necessary for US healthcare revenue cycle management (RCM) and remittance mastery. Financial correctness & Numerical Precision: Exceptional speed and 100% correctness in batch balancing, contractual write-off computations, line-item payment distribution, and numerical data entry.
